If your responses to our initial screening application indicate that your gametes have a high probability of genetic desirability, our Yuba City, CA office will be mailing to you a preliminary application. Please complete this and mail it back to the address found on the form. Upon review and acceptance of this form, a formal and complete application will be mailed to you for you to complete and return with your photos.
